Back-to-School Forms for New and Returning Students (Grades K-12)

Back-to-school forms for the 2021-2022 school year are available in the PowerSchool Parent Portal


Please note that these forms include the optional Technology Protection Plan for students in grades K-12, as all students will be taking home a device throughout the school year. If you select to purchase this optional protection plan for your student, please pay the $25 fee in SPS EZpay.


NEW STUDENTS

Parents/guardians of all new students will need to access technology forms for their student’s district-issued device, including the optional Technology Protection Plan. Please complete the technology forms before the first day of the school year (August 18, 2021). 


RETURNING STUDENTS

Parents/guardians for all returning students will need to access and complete the forms in PowerSchool to verify and update important student information before the first day of school.  These forms involve parent/guardian contacts, emergency contacts, medical history and all technology forms associated with students’ school-issued devices.  Please complete these forms before the first day of the school year (August 18, 2021).  


Please follow the following steps to complete these forms:

  1. Log in to your PowerSchool parent account. If you need help with your log-in information, or you have not set up a PowerSchool parent account, please contact [email protected].

  2. Once you are logged in to your PowerSchool parent account, select the “Forms” link in the left navigation column. 

  3. Individually select and verify or complete ALL of the required forms under the General Forms tab. Be sure to click the Submit button at the bottom of each form to save your work.

  4. If you have multiple students associated with your PowerSchool parent account, please click on each student in the tabs at the top of the webpage and verify or complete ALL of the required forms for each of your students.


Student Fees (K-12)

Families of new and returning students can log in to SPS EZpay to pay any fees associated with their students. At this time, the fees available for payment include instructional fees for elementary and middle school students (K-8) and the optional Technology Protection Plan fee ($25) for students in grades K-12. Families wishing to purchase the Technology Protection Plan must do so before the first day of the school year (August 18, 2021). Please read the details of the protection plan on our website.

 

Additional fees for athletics and high school courses, and fees for senior parking will be available at a future date. 


New to SPS EZPay? View these instructions on how to sign up for SPS EZPay. 


To add a student to your existing EZPay account: Please click on My Account and then Add a Family Member and provide the student’s district-issued student ID number and last name. You can find your child’s six-digit student ID number by logging into the PowerSchool Parent Portal.


If you have any questions about student fees or accessing your EZpay account, please contact [email protected]. If paying these school fees will cause a hardship for your family, please contact your child’s principal or school counselor.

School Lunches

Thanks to a USDA waiver, all students enrolled in Upper Arlington Schools (preK-12) will be able to receive free lunch this school year. At the high school and middle schools, grab-and-go breakfast will also be available for no charge, and a la carte options will still be charged to students’ EZPay accounts. We will communicate with families in advance of any changes to this temporary federal program.

Device Swap and Distribution (Grades K-4 and new students)

Grades 1-4 

Students currently in grades 1-4 who have a district-issued iPad will bring it to school during the first week of school and will swap out for a new iPad 6th generation device. They will set up their new device in the classroom with their teachers, the media specialists and building technicians.

 

Students new to the district

  • Kindergarten and elementary students new to the district will receive their device during the first week of school in their classrooms.  

  • Middle school students new to the district can pick up their device from the media centers during the first week of school.

  • High school students new to the district can pick up their device from the first-floor tech office during the first week of school.
