Back-to-School Forms for Returning Students (Grades 1-12)

Back-to-school forms for the 2022-2023 school year are available in the PowerSchool Parent Portal


Please note that these forms include the optional Technology Protection Plan for students in grades K-12, as all students will be taking home a device throughout the school year. Those wishing to purchase the optional Technology Protection Plan must complete the technology forms in PowerSchool AND pay the $25 fee in SPS EZpay before the first day of school for the 2022-2023 school year (August 16, 2022). If the $25 fee is not paid by the first day of school, the plan is void.


RETURNING STUDENTS

Parents/guardians for all returning students will need to access and complete the forms in PowerSchool to verify and update important student information before the first day of school.  These forms involve parent/guardian contacts, emergency contacts, medical history and all technology forms associated with students’ school-issued devices.  Please complete these forms before the first day of the school year (August 16, 2022).  


Please follow the following steps to complete these forms:

  1. Log in to your PowerSchool parent account. If you need help with your log-in information, or you have not set up a PowerSchool parent account, please contact [email protected].

  2. Once you are logged in to your PowerSchool parent account, select the “Forms” link in the left navigation column. 

  3. Individually select and verify or complete ALL of the required forms under the General Forms tab. Be sure to click the Submit button at the bottom of each form to save your work.

  4. If you have multiple students associated with your PowerSchool parent account, please click on each student in the tabs at the top of the webpage and verify or complete ALL of the required forms for each of your students.

Please note that back to school forms will need to be completed through the PowerSchool link provided. The PowerSchool app will remain unavailable until the start of the school year.

Student Fees for New and Returning Students (K-12)

Families of new and returning students can log in to SPS EZpay to pay any fees associated with their students. At this time, the fees available for payment include: 


  • Instructional fees for elementary and middle school students (K-8);

  • Middle school optional community building / social fees; 

  • Middle school global language workbook fees; and

  • The optional Technology Protection Plan fee ($25) for students in grades K-12.

Families wishing to purchase the optional Technology Protection Plan must complete the technology forms in PowerSchool AND pay the $25 fee in SPS EZpay before the first day of school for the 2022-2023 school year (August 16, 2022). If the $25 fee is not paid by the first day of school, the plan is void.

Additional fees will be available at a future date — including: middle school Physical Science fees; high school course fees; fall athletic fees; and possible band / orchestra fees. Please allow 48 hours from the time of course enrollment to see fees updated in EZPay.

To add a student to your existing EZPay account: Please click on My Account and then Add a Family Member and provide the student’s district-issued student ID number and last name. You can find your child’s six-digit student ID number by logging into the PowerSchool Parent Portal.

If paying these school fees will cause a hardship for your family, please contact your child’s principal or school counselor.

Updates on school meals

Due to the pandemic, the federal government has been providing universal free school meals for students.  This program has allowed Upper Arlington Schools and other school districts to provide free lunches to all students since the spring of 2020.


This federal waiver program expired this summer.  This means that beginning with the 2022-2023 school year, free or reduced-price meals will be available only to the families that qualify based upon financial need. The free or reduced-price meal benefit application is available in the Titan Family Portal.


All other students will be charged for meals and a la carte items / snacks purchased through the school cafeteria.


The meal costs for the start of the 2022-2023 school year will be:


  • Elementary lunch: $3.00

  • Middle school breakfast: $1.75; middle school lunch: $3.25 and $3.75

  • High school breakfast: $2.00; high school lunch: $4.25 and $4.75

Device distribution information

Grades K and 9, and all new students
  • Kindergarten, elementary and middle school students new to the district will receive their device during the first week of school in their classrooms.  
  • Incoming 9th grade students will swap their 6th generation iPad from middle school on their first day/week of school for the newer 8th generation iPad that HS students use. This exchange will be coordinated with freshman activities scheduled at the high school during the day.
  • High school students new to the district can pick up their device from the first-floor tech office during the first week of school.
