Back-to-School Forms for Returning Students (PreK and Grades 1-12)

Back-to-school forms for the 2023-2024 school year will be available in early August in the PowerSchool Parent Portal


Parents/guardians for all returning students (preK and grades 1-12) will need to access and complete the forms in PowerSchool to verify and update important student information before the first day of the school year on August 16, 2023.  These forms involve parent/guardian contacts, emergency contacts, medical history, student permissions and all technology forms associated with students’ school-issued devices, including the recently updated Acceptable Use Policy and the optional Technology Protection Plan.Those wishing to purchase the optional Technology Protection Plan must complete the technology forms in PowerSchool AND pay the $25 fee in SPS EZpay before the first day of school for the 2023-2024 school year (August 16, 2023). If the $25 fee is not paid by the first day of school, the plan is void.


The PowerSchool Parent Portal will remain closed to families until the forms are available in early August. At that time, you can follow the following steps to complete the forms: 


  1. Log in to your PowerSchool parent account. If you need help with your log-in information, or you have not set up a PowerSchool parent account, please contact [email protected].

  2. Once you are logged in to your PowerSchool parent account, select the “Forms” link in the left navigation column. 

  3. Individually select and verify or complete ALL of the required forms under the General Forms tab. Be sure to click the Submit button at the bottom of each form to save your work.

  4. If you have multiple students associated with your PowerSchool parent account, please click on each student in the tabs at the top of the webpage and verify or complete ALL of the required forms for each of your students.

Please note that back to school forms will need to be completed through the PowerSchool link provided. The PowerSchool app will remain unavailable until the start of the school year.

Student Fees for New and Returning Students (K-12)

Families of new and returning students can log in to SPS EZpay beginning in early August to pay any fees associated with their students. The fees available for payment will include: 


  • Instructional fees for elementary students (K-5);

  • Middle school optional community building / social fees; 

  • The optional Technology Protection Plan fee ($25) for students in grades K-12.

Families wishing to purchase the optional Technology Protection Plan must complete the technology forms in PowerSchool AND pay the $25 fee in SPS EZpay before the first day of school for the 2022-2023 school year (August 16, 2022). If the $25 fee is not paid by the first day of school, the plan is void.

Middle school and high school course fees will be available in September. Fall athletic fees and UAHS parking fees will be available after those assignments are completed by the schools. Please allow 48 hours from the time of course enrollment to see fees updated in EZPay.


New to SPS EZPay? View these instructions on how to sign up for SPS EZPay. Please note that any families of students who are new to the district for the upcoming school year will be able to sign up for SPS EZPay beginning on August 1.


To add a student to your existing EZPay account: Please click on My Account and then Add a Family Member and provide the student’s district-issued student ID number and last name. You can find your child’s six-digit student ID number by logging into the PowerSchool Parent Portal.
